In a small saucepan over medium heat, add milk; simmer and remove from heat. Add milk to a blender; cover and blend on medium until frothy. Immediately top hazelnut coffee with froth.
Swirl chocolate sauce in a spiral on top of froth. Drag a toothpick through the froth several times, starting at the center of the mug and pulling towards the rim.
