What to Bring to Your Vaccination Appointment
- Insurance card* and Medicare Part B red, white and blue card (if Medicare recipient)
- Photo ID (recommended)
- COVID-19 vaccination record card (if applicable)
- A mask to wear that covers your mouth and nose
*Only required for those enrolled in health insurance. Those who are not covered by insurance can still receive the COVID-19 vaccine at no cost.
**Individuals receiving an additional dose (for immunocompromised) of the COVID-19 vaccine will need to self-attest to their eligibility. Individuals will NOT be required to show documentation to prove eligibility.
Safety Measures at Hy-Vee
Hy-Vee’s vaccination services will continue to be conducted in compliance with the “CDC’s Guidance for Immunization Services During the COVID-19 Pandemic” for the safe administration of vaccines. For example, Hy-Vee Pharmacy members are required to wear masks when administering vaccines. All patients must also wear masks during the vaccination process and are encouraged to practice social distancing when at the pharmacy location.
Hy-Vee continues to follow CDC guidelines for safety and precautions. Hy-Vee’s stores, including its pharmacy areas, maintain enhanced cleaning regimens, placing extra emphasis on high-touch surfaces like door handles, PIN pads at the checkout, and more.
According to the CDC, patients can expect to experience mild side effects from the vaccines. Common side effects include pain or swelling in the arm where the vaccine was administered, fever, chills, tiredness and headache. Symptoms should be temporary and are expected to go away in a few days.

If redness or tenderness where you received the vaccine increases after 24 hours, or if you are worried about any other side effects, contact your health care provider immediately.
