Diabetes Self Managmenet Education and Training

Whether you just found out you have diabetes or have been living with it for some time, our team will help you achieve blood sugar control and learn about how to self-manage your diabetes and understand the AADE 7 Self-Care Behaviors™ as listed below.

Course Outline:

  • Monitoring: Learn about timing, frequency, target values and interpreting results of blood glucose monitoring.
  • Healthy Eating: Gain knowledge about the effect of food on blood glucose, meal planning, label reading, food preparation, portion control and carbohydrate counting.  Enjoy personal shopping as you tour the aisles with your registered dietitian/certified diabetes educator.
  • Being Active: Collaborate to address barriers and develop an appropriate plan for physical activity to improve overall fitness, manage weight and improve blood glucose control
  • Taking Medications: Meet with a clinical pharmacist to review each medication, it’s action, side effects, efficacy, toxicity, prescribed dosage and appropriate timing of administration.
  • Problem Solving: Sharpen problem solving skills to be able to make informed decisions about food, activity and medications.
  • Reducing Risks: Learn skills to reduce risks such as smoking cessation, foot inspections, blood pressure monitoring, self-monitoring of blood glucose, immunizations and more.
  • Healthy Coping: Learn how psychological distress impacts motivation to manage diabetes. Receive support and guidance in working through obstacles.

Follow-up

Follow-up consults will be performed to ensure success and that all diabetes related goals are being met. A consultation report will be sent to the patient’s referring physician.
