1. ELIGIBILITY: Open only to legal residents of IA, IL, KS, MN, MO, NE and SD who are 18 years of age or older at the time of entry. Officers, directors, employees, agents of Sponsor, any company affiliated with Sponsor, any company affiliated with this promotion (Hy-Vee and each of their respective affiliates, subsidiaries, advertising and sweepstakes agencies and suppliers involved in this sweepstakes, collectively “Promotional Participants”), as well as immediate family members and/or those members of the same household of any of the above are not eligible to participate or win. Void outside IA, IL, KS, MN, MO, NE and SD and where prohibited by law.
2. TWO WAYS TO ENTER:
A) Online Method of Entry: Beginning 12:00:01 a.m. Central Time (CT) on 8/23/09 through 11:59:59 p.m. (CT) on 9/05/09, log on to www.hy-vee.com and follow the on-screen instructions to complete the on-line entry form.
B) Mail-In Method of Entry: Beginning 8/23/09 until 9/05/09, you may mail-in an entry by hand printing on a plain piece of 3” x 5” paper your complete name, street address (no P.O. Boxes), city, state, ZIP code, date of birth (mm/dd/yyyy), plus daytime and evening telephone numbers. You must mail your entry in a hand-addressed, business-size (#10) envelope with first-class postage affixed, to: be Hy-Vee “Be vitmainwater10’s Multitask Master” Sweepstakes, P.O. Box 27175, Golden Valley, MN 55427. Entries by mail must be postmarked by 9/05/09 and received by 9/12/09. Limit one (1) entry per stamped envelope. No mechanically reproduced entries permitted. Illegible or incomplete entries are void.

3. RANDOM DRAWING: Potential winners will be selected in a random drawing held on or about 9/15/09 from among all eligible entries received via Rules 1A and 1B. Potential winner will be notified by phone or mail. Odds of winning will depend upon the number of eligible entries received via Rules 1A and 1B. Limit one (1) prize per person/household.
4. PRIZES & APPROXIMATE RETAIL VALUES (“ARV”): (1) Grand Prize - the Grand Prize consists of a $2,500 Sponsor-specified housecleaning services Gift Card; one (1) laptop computer awarded in the form of a $800 Sponsor-specified electronics retailer Gift Card; (1) one personal chef experience, awarded in the form of a $500 American Express Gift Card; and a 1-year supply of vitaminwater10™ consisting of 12 cases of 20 oz. low-cal all natural vitaminwater10™, each case contains 24, 20 oz. bottles of vitaminwater10™; flavors are at the discretion of Sponsor. Gift Cards are not redeemable or exchangeable for cash and are subject to all terms and conditions of use as established by issuer. Sponsor not responsible for lost or stolen Gift Card. Prizes consist of only those items specifically listed as part of the prize. No transfer, assignment, substitution, or cash redemption of any prize will be allowed. 5. GENERAL RULES: Sponsor is not responsible for lost, late, damaged, delayed, mutilated, postage due, misdirected entries or entries not received by the entry deadline, interrupted or unavailable network, server or other connections, miscommunications, failed telephone, computer hardware or software, and other technical errors or failures of any kind, including any injury or damage to any person’s computer related to or resulting from participating in or experiencing any materials in connection with the sweepstakes; unauthorized human intervention, failed, incomplete, garbled, jumbled or delayed computer transmissions or other errors of any kind, whether human, mechanical or electronic, that may limit a user’s ability to participate in the sweepstakes, including without limitation, the incorrect or inaccurate capture of entry information online regardless of cause. All materials become exclusive property of Sponsor and will not be acknowledged or returned. Any use of robotic, automatic, programmed or like entry methods will void all entries affected by such methods and be deemed tampering. Sponsor reserves the right to prohibit the participation of an individual if fraud or tampering is suspected, or if the individual fails to comply with any requirement of participation as stated herein or with any provision in these Official Rules. INTERNET CAUTION: ANY ATTEMPT BY AN INDIVIDUAL TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS SWEEPSTAKES IS A VIOLATION OF CRIMINAL AND CIVIL LAWS, AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH INDIVIDUAL TO THE FULLEST EXTENT OF THE LAW. Sponsor reserves the right to cancel or modify the online portion of the Sweepstakes if fraud, misconduct or technical failures destroy the integrity of the Sweepstakes or if a computer virus, bug, or other technical problem corrupts the administration, security, or proper administration of the program as determined by Sponsor/judging agency/administrator, in their sole discretion. In the event of termination of the online portion, a notice will be posted online and the random drawing will be conducted from among all eligible entries received prior to the time of termination and all eligible entries received via the mail-in method of entry. In the event a dispute arises regarding the ownership of any e-mail account, entry will be declared made by the “authorized e-mail account holder” and any damage made to the Web site will also be the responsibility of the authorized e-mail account holder of the e-mail address submitted at the time of entry. Entrants are not allowed to share e-mail addresses. “Authorized e-mail account holder” is defined as the natural person who is assigned to an e-mail address by an Internet access provider, online service provider or other organization (e.g., business, educational institution, etc.) that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address. Participant may be requested to provide Sponsor with proof that the participant is the authorized e-mail account holder of the e-mail address associated with the account/submission. Proof of submitting entries will not be deemed to be proof of receipt by Sponsor. All federal, state and local taxes are the sole responsibility of winner. Grand Prize winner will be issued a IRS form 1099. All federal, state and local laws and regulations apply. Potential Grand Prize winner must sign and return an Affidavit of Eligibility and Release of Liability/Publicity Release Form within fourteen (14) days of notification. Noncompliance within this time period may result in disqualification and an alternate winner may be selected. Return of any prize/prize notification as undeliverable may result in disqualification and an alternate winner may be selected. Acceptance of prize constitutes permission to the Sponsor and its Promotional Participants to use winners’ names and/or likenesses for purposes of advertising and trade without further compensation, unless prohibited by law. By accepting prize, winners agree to release and hold Sponsor, the Promotional Participants and their respective directors, officers, employees and assigns, harmless against any and all claims, damages, losses, expenses and liability arising out of use/acceptance of prize. Entrants assume all liability for any injury or damage caused, or claimed to be caused, by participation in this sweepstakes or use or redemption of prize. By participating in this sweepstakes, entrants agree to be bound by the Official Rules and the decisions of the judges. Sponsor and its Promotional Participants are not responsible for any typographical or other error in the printing, the offering or the announcement of any prize or in the administration of the sweepstakes.
